PLANO COMMUNITY UNIT SCHOOL DISTRICT 88: Many Opportunities for Students to Get Low Cost Physicals & Vaccines

Plano Community Unit School District 88 issued the following announcement on July 3.

WALK IN CLINIC AT PLANO SCHOOL DISTRICT

Plano School District will be hosting a medical clinic for students that need school or sports physicals. This walk-in clinic will be at Plano High School on August 1, 2, and 3, 2018. Local physicians and medical assistants will be available and will accept the medical card and most major insurances. For those paying cash, the cost will be $25.

WHO NEEDS A SCHOOL OR SPORTS PHYSICAL?

All students entering Preschool, Kindergarten, 6th, and 9th grades will require a current school physical exam on file (on the State of Illinois - Certificate of Child Health Examination form). Students who plan on playing sports are also required to have a current sports physical on file (IHSA/IESA Pre-Participation Examination form). These forms are available in “Documents” in the online registration or on our website plano88.org under the Parents and Students tab - Health Services Information/Forms.

VACCINES

If your student requires immunizations and qualifies under Illinois Vaccine for Children Program (VFC) eligibility requirements (18 years or younger, enrolled in Medicaid under Title 19 uninsured, and under-insured) please Click here for the English version of the Kendall County Health Department Back to School Shot Clinic flier or Click here for the Spanish Version.
