Oswego Community Unit School District 308 issued the following announcement on Aug. 1.
Attention Parents/Guardians of 12th Grade Students
All students entering their Senior year of High School, grade 12, are required to show proof of the Meningococcal vaccine (MCV4) after their sixteenth birthday. Seniors will not be permitted to pick up their schedule or start school without proof of this vaccine.
If your student does not have the vaccine it can be obtained by contacting your health care provider, a local pharmacy or the Kendall County Department of Health at 630-553-9100.
Attention Parents/Guardians of 9th Grade Students
All students entering their freshman year of High School, grade 9, are required to show proof of a physical examination dated August 15, 2017 or later. Freshmen will not be permitted to pick up their schedule or start school without proof of a current physical exam.
If your student does not have the Exam it can be obtained by contacting your health care provider, a local pharmacy clinic or the VNA Health Center of Aurora at 630-892-4355.
Attention Parents/Guardians of 6th Grade Students
All students entering 6th Grade are required to show proof of a physical examination dated August 15, 2017 or later and proof of the Tdap vaccine after age seven and the Meningococcal vaccine (MCV4) on or after their eleventh birthday. Sixth grade students will not be permitted to start school without proof of a current physical, the Tdap and Meningococcal vaccine.
If your student does not have the Exam or immunizations, these can be obtained by contacting your health care provider, a local pharmacy clinic, or the VNA Health Center of Aurora at 630-892-4355. Immunizations are also available through the Kendall County Department of Health at 630-553-9100.
